Dental Services Survey

The Heart of Birmingham PCT is conducting research into the experience of Dental Services locally. This is with a view to piloting improvements to the services in this area.

To help them to take this initiative forward a survey is being undertaken and it would be really helpful if you could make time to complete the attached short questionnaire. Remember that this is all part of the way to improve services in this local area.
